What you are agreeing to
Redux Rehab Pty Ltd (ABN 36 696 717 432) is a workplace rehabilitation provider approved by the State Insurance Regulatory Authority (SIRA) for the NSW workers compensation scheme (Same Employer stream). We also provide rehabilitation services under the NSW CTP (motor accident) scheme.
You are completing this form because you have spoken with Redux Rehab or your referrer and verbally agreed to Redux Rehab acting as your rehabilitation provider. Signing below makes that agreement formal and allows your program to start.
Our role is to plan and coordinate your recovery at work: assessment, recovery at work planning, and communication with your treating doctor, your employer and your insurer. We do not replace your treating doctor or your other treatment providers, and your choice of treating team stays yours.
Your right to choose your provider
You have the right to choose your own rehabilitation provider. For a NSW workers compensation claim you may choose any SIRA approved workplace rehabilitation provider, and Redux Rehab is one available option. There is no obligation to proceed with us, and your decision will not affect any service, entitlement or relationship with your referrer, employer or insurer.
You can change provider at any time by telling your insurer or case manager.
Conflict of interest disclosure
Redux Rehab Pty Ltd is jointly owned by its two directors: Andrew Ellis, who also owns PACT Physio & Rehab (PACT Health Pty Ltd), and David Parkinson, who also owns Vital Axis Health.
If your referral to Redux Rehab comes from PACT Physio & Rehab, from Vital Axis Health, or from a practitioner at either practice, that ownership is a conflict of interest. In line with the conflict of interest policy in Redux Rehab's SIRA approval, this conflict is advised to all parties at the time of referral and recorded in Redux Rehab's conflict of interest register.
The conflict is managed at arm's length: the businesses are entirely separate legal entities with separate finances and systems, no referral fees, commissions or incentives of any kind exist between them, and the director connected to the referring practice takes no part in the clinical management of your case.
You remain free to choose any other provider, now or at any time.
Your information and your privacy
To deliver your program we collect personal and health information relevant to your rehabilitation: your contact details, employment details, claim details, and information about your injury, treatment and capacity.
With your consent, we share relevant information with the people supporting your claim: your insurer or scheme agent, your treating doctor and treating practitioners, your employer (limited to what supports your recovery at work, such as certified capacity and suitable duties, never your broader clinical information), and SIRA where the law requires.
We handle your information under the Privacy Act 1988 (Cth), the Health Records and Information Privacy Act 2002 (NSW) and our Privacy Policy at reduxrehab.com.au/privacy-policy. Your records are stored securely in Australia and retained for at least 7 years from your last service, as the law requires.

Participation, costs and stopping
Your participation is voluntary. You can withdraw your consent at any time by telling us in writing. If you withdraw, we stop providing services and advise your insurer, as the scheme requires.
Rehabilitation services under workers compensation and CTP are billed to the insurer. There is no cost to you.
This form and your signature are collected electronically. A signed PDF copy is stored on your confidential patient record, and you can ask us for a copy at any time.
